Community Physiotherapist

Band 6 Community Physiotherapist - Croydon Health Services. The successful candidate will deliver physiotherapy services in the community, supporting patients in their own homes or care settings as part of a multi-disciplinary team.

This is a full-time role for an initial 6-month period with potential for extension, subject to service demand and performance.

Key Responsibilities:

• Provide physiotherapy assessments and treatment to patients in the community setting

• Work autonomously and as part of a wider multi-disciplinary team

• Maintain accurate and up-to-date patient records in line with NHS standards

• Plan and implement individual treatment programmes for patients with a variety of conditions

• Contribute to discharge planning and onward referral where appropriate

• Liaise with other healthcare professionals, patients, carers, and families to ensure continuity of care

• Travel between patient homes and community bases as required

Essential Requirements:

• Previous experience in a community physiotherapy role

• Valid HCPC registration as a Physiotherapist

• UK clinical experience at Band 6 level or equivalent

• Car owner and willing to drive for work purposes

• Able to work under the Direct Engagement model
