Community Care Worker


Job Summary:

We are seeking a compassionate and dedicated Community Support Worker to join our team in Guernsey . The ideal candidate will provide essential support to adults 18+ in the community, offering assistance to vulnerable individuals and families in need. This role requires someone who is empathetic, resourceful, and passionate about making a positive impact on people's lives.



Key Responsibilities:

  • Case Management: Assess, plan, and monitor the care and support needs of clients, ensuring they have access to the necessary resources and services.

  • Crisis Intervention: Provide immediate support to individuals in crisis, offering guidance and assistance during challenging situations.

  • Community Outreach: Engage with at-risk populations, including elderly, disabled individuals, and families facing hardship, to offer support and guidance.

  • Home Visits: Travel across Guernsey to visit clients in their homes, providing assistance with daily living tasks and ongoing care needs.

  • Resource Navigation: Help clients connect with local services and resources, such as housing, health care, and social services.

  • Advocacy: Support clients in accessing their rights and opportunities within the community, ensuring their needs are heard and met.



Key Skills & Qualifications:

  • Experience: Previous experience in social services, community support, or case management is preferred.

  • Empathy & Compassion: A genuine desire to help others and provide meaningful support to those facing personal or social challenges.

  • Communication: Strong interpersonal skills, with the ability to build rapport with clients, families, and other professionals.

  • Problem-Solving: Ability to assess complex situations and offer creative, effective solutions.

  • Confidentiality: Understand and adhere to the importance of client confidentiality and data protection regulations.

  • Transportation: A reliable vehicle is required for travel across Guernsey to visit clients in their homes and other community locations.

  • Flexibility & Adaptability: The role requires the ability to adapt to changing situations and client needs.
