Experienced Adults Social Worker - Mental Health (Transitions Team) Tameside Council | £35 per hour | Full-time, Temporary Are you an experienced Adults Social Worker with a strong background in mental health? About the Role We are looking for a dedicated and skilled Adults Social Worker to join our Transitions Team within a local East Manchester Council. This role focuses on providing a comprehensive social care service to adults of working age experiencing moderate to severe mental health problems within the community. You will be responsible for: Conducting Care Act assessments and developing clear support plans Working with young people transitioning from children's to adult mental health services Completing funding applications and legal documentation (CoPDoL, DoLS, 21a's, court reports) Engaging with individuals and families who may be difficult to reach Supporting carers in line with the Care Act Collaborating with multi-disciplinary teams to deliver person-centred care What We're Looking For Qualified Social Worker with relevant registration (e.g., Social Work England) Strong experience in mental health social work Familiarity with Liquid Logic case management system Experience in transitions work (working with young people moving to adult services) Knowledge of legal frameworks such as DoLS, CoPDoL, and the Care Act Working Hours & Flexibility Monday to Friday, 09:00 - 17:00 (some flexibility required for planned interventions) If you're an experienced mental health social worker looking for a rewarding role with a great hourly rate, apply today!
