Social Worker - Hospital Discharge Team, Manchester Council

Randstad is currently recruiting for a dedicated Social Worker to join the Hospital Discharge Team at Manchester City Council . This is a full-time locum role offering up to 30.00 per hour , providing an excellent opportunity to work within a fast-paced, person-centred environment that focuses on timely and safe discharges from hospital settings.

About the Role

As a Social Worker in the Hospital Discharge Team, you will work closely with health professionals, families, and community services to coordinate discharge planning. Your role is to ensure that individuals leaving hospital receive the right care and support to recover safely, reduce hospital readmissions, and promote long-term wellbeing.

Key Responsibilities

  • Undertake Care Act assessments in hospital settings or virtually.
  • Collaborate with NHS colleagues to support safe and timely discharges.
  • Develop person-centred care and support plans, involving service users and their families in decision-making.
  • Arrange appropriate packages of care, residential placements, or community-based support where required.
  • Assess mental capacity where relevant and complete best interest decisions.
  • Record case work accurately and maintain compliance with statutory duties and local procedures.

Requirements

  • Registration with Social Work England
  • Qualified Social Worker with a Social Work Degree or equivalent
  • Enhanced DBS check
  • Previous experience in hospital discharge, reablement, or community adult social care
  • Knowledge of the Care Act 2014 , Mental Capacity Act , and discharge to assess models
